How to Book your child's place on one of our Bishop Gilpin after school clubs

Step 1/5: Fill in our Registration form for our after school clubs

You can fill in the form below on this website or by emailing info@teamupmultisports.co.uk if you would rather write the answers out in email format. Those that have booked within the last year can fill out the shorter continuing form on request/when enquiring via email.

Tips!

  • Type an answer in every box, even those that do not apply by saying none or something similar.

  • There is a suggestion or help within each box to prompt the type of answer we are looking for. This disappears when you start typing.

  • Click submit after you have answered each question.

  • If successfully completed, the form will disappear and be replaced by a message saying The form has been successfully sent. If this does not happen check each answer box to see if there is a message saying This field is required then type an answer in that box and click submit again.

  • If your internet is having problems it may say something has gone wrong and ask you to try again later.

  • If you do not know where you are going wrong, please check step 2.

Step 2/5 (If Step one was successful continue to step three)

If you are having trouble filling in the form online, please type your email address and phone number and click submit and we will reply within 48 hours.

Step 3/5

If you have successfully submitted the form, we will email you on the email address you provided within 48 hours.

If you have not received an email from us within this time please check your junk file before emailing info@teamupmultisports.co.uk

If we have spaces available we will then send you our payment details and cost of the block booking period. We require payment via an online bank transfer. If this is not possible, please let us know and we will sort an alternative method.

If we are already fully booked we will email you to let you know your child is on the waiting list and will let you know when the space opens up.

Step 4/5

Once we have received the payment we will send a confirmation email. This notifies you that your child is on our register. Without this confirmation you should assume your child is not registered. We will never take a child to our session if they are not on our register to ensure their safety in case of emergency.
